Dr. Siegel's Payment Policy Please note: Dr. Siegel's office does not participate in any HMO or PPO insurance services
We respectfully request that you pay for all dental services in our office, at the time that these services are renderred. We accept only personal forms of payment, including personal checks, VISA and MaterCard. We do not accept dental insurance or PPOs/HMOs.
As a service to our patients, we will be happy to assist you with completing and mailing your insurance forms for your own reimbursement. Payment is still requested at the time of service.
